 REGISTRATION INSTRUCTIONS

Step 1:

Print a registration form,

or pick one up from Hermitage Agway (at the back desk).

To print a registration form:

click here Player registration .pdf format for an application in Adobe Acrobat format.  (hint: from your web browser choose File from the upper left corner, then either save the file to your hard drive or use the print command.)

Step 2:

 Complete the information in the data bases and sign the consent.  Please use the chart at the bottom of the form to calculate your payments, and don't forget to complete the player ID # !!!!

Step 3:

For Shenango Valley United travel teams:

Enclose a photocopy of your state certificate of birth and a 1 x 1 inch facial photo (clip a school wallet picture).  IF your child will be on a team more than 2 years above their age, the Pa West consent to play up must be completed and notarized, and enclose with the application.

            FOR TRAVEL TEAMS ONLY: If you reside in Ohio, you must register with Ohio North and obtain their permission to play out of state. Before you mail ANY material, please contact the club registrar for guidance by sending an email to:  EMAIL@SAYSA-PA.ORG  The club  fee for Ohio travel players is reduced to $61 to accommodate the Ohio North fees.

For Shenango Valley Soccer recreational program the photo is not needed, but the birth certificate is. Please mark which practice group you want your child to attend:  Sharon, Sharpsville, or Hermitage.

Step 4:

Enclose the appropriate payment by check or money order-NO CASH!:

Fundraising exemption:

Families may exempt out from the annual spring fundraising event with a donation of $25 per family. If you do not mark the exemption box and attach the donation, your family is expected to participate in the board approved fundraising event during the spring session.

Shenango Valley United travel team:

$72 per player club fee

$20 if a new travel jersey is needed (please mark the form accordingly)

Shenango Valley Soccer recreational program:

$37 for first child per household; $32 for second child, $257for third and subsequent child.

If you do need a new rec jersey, add $7.50 for that child (please mark the form accordingly)

Step 5:

Mail the form to the address indicated on the form:

Step 6:

You will be contacted by the registrar when your application is received either by email, phone, or postcard.  Your coach will contact you about one week before the first practice regarding practice times and days.
