Effective August 1, 2007 Pa West requires that every 2 years, ALL VOLUNTEERS submit EITHER the clearance forms from the Commonwealth of Pa, OR a Kidsafe clearance through Pa West and the USYSA. The disclosure forms which Pa West used in the past are no longer valid.

If you have current Child abuse and State Police Criminal background clearances from a current job, those may be copied and sent to the club to be on file.
To apply for the formal background checks, use the following process:
#1) The Pa State Police criminal background check can now be done on line: https://epatch.state.pa.us/Home.jsp or the traditional method by mail.
This can be printed by opening this file: sp4-164.pdf -just print the form, complete it, have the report mailed to your home address, mail the request to the indicated address. When you receive the report, provide the club with a copy of the report.
#2) Also required is a Childline report, which can be printed by opening this file: DPWchildabuse.pdf. Instructions are available by opening: Pennsylvania Child Abuse History Clearance Instructions. Follow the same process as with form #1.
